Are there any risk factors that can lead to a challenging PAT experience, also known as a “bad trip”?

While considered to be safe overall, psychedelics can sometimes leave people vulnerable to challenging experiences, particularly if there is a lack of readiness for a journey or if the psychedelic is improperly administered. This can include adverse reactions like feelings of anxiety or panic, extreme fear, ego dissolution (reduction in self-identity), or disturbing visions. 1 It is worth noting, however, that such challenging PAT experiences, with proper integration, can lead to valuable and insightful lessons that can be used to make meaningful changes in one’s life.2 When exploring psychedelic medicines, it is important to know the risk factors to consider that may make individuals more susceptible to a challenging PAT experience, including: 3, 4  

  • Lack of preparation, which is an essential part of PAT, as it helps individuals to mentally prepare for their psychedelic experience. Preparation involves the individual and their provider meeting prior to the administration of the psychedelic medicine to discuss concerns, understand what to expect, and to set intentions for the experience.
  • High dosage of a psychedelic substance.
  • Difficult, unresolved past experiences and/or trauma.
  • Working with providers who are inexperienced in PAT and don’t provide the necessary support during and after treatment.
